• Patient Care Advocates– These VFH staff will help patients across the continuum of care beginning with finding a provider, helping patients schedule and coordinate transportation to appointments, and navigating the health care system, including health insurance issues. Patients can chat with an advocate via an online chatroom to preserve anonymity. (We will need a developer to help us create this). We will find patients/clients via our webinars/workshops (below) and via social media ads, Google ads, and outreach. to hospitals. If a patient is unable to reach out for help on their own, Patient Advocates will work with a caregiver to coordinate treatment.
  • Webinars and Online Workshops– These sessions are part of a “Patient Empowerment Program” that meets prospective patients where they are and give them the tools and resources to take the first step in seeking treatment.
  • Patient Support Network– Online forum where patients with stigmatized conditions can support each other and discuss their issues anonymously.
  • Patient Empowerment Grants– These need-based grants will be available to help patients pay for treatment that would otherwise be out of reach for them.
